Change of address
If you have a change of address, marital status, etc. you may need to complete the Notice of Demographic Change (PMRB-5a).

Beneficiary Changes
Your beneficiaries may be changed at any time – even during retirement. Make sure you check your annual statement for listed beneficiaries for accuracy. Complete a Nomination of Beneficiary (PMRB-2) form, complete it and send it to PMRS.
