Enrollment
- Employees who meet the eligibility requirements of their employer’s pension plan are enrolled (or re-enrolled) in the plan to become PMRS Members. This is accomplished by completing the Member Enrollment Application (PMRB-1).
- Membership in the plan for eligible employees is required unless otherwise indicated in the plan document.
- Upon enrollment, you should receive a Benefit Summary that addresses the provisions specific to you plan.
Beneficiaries
- A beneficiary is a person or entity designated by a member to receive benefits not payable to a survivor annuitant after the death of the member.
- Upon enrollment in the pension plan, you will name your beneficiaries by completing the nomination of beneficiaries’ form (PMRB-2).
- Your beneficiaries may be changed at any time – even during retirement. Make sure you check your annual statement for listed beneficiaries for accuracy.

Contribution Changes
- If your plan allows, you may make changes to your optional contributions. To do this, you and your employer need to complete the Voluntary Contribution Change (PMRB-15).

Death
- Depending on the provision of your plan and the status of your account at the time of death, a benefit may be payable to a beneficiary or survivor annuitant.
- If a member has died, please contact our office.
Demographic Changes
- If you have a change of address, marital status, leave of absence (Military, maternity/paternity leave, etc.) You may need to complete the Notice of Demographic Change (PMRB-5a) or Notice of Service Record Change (PMRB-5b).

Disability
- Some pension plans provide up to two types of disability retirement benefits: Service Connected and Non-Service Connected.
- The disability caused by sickness or injury must be total, it must be permanent, and it must prevent you from performing the material and substantial duties of any occupation for which you are reasonably qualified by education, training, or experience that would maintain your pre-disability standard of living.
- If you become disabled, contact our office to start the Disability Retirement application process.
Separation of Service
This includes voluntary or involuntary termination of employment, disability or retirement, unless otherwise identified by your employer as a leave of absence.
- If you leave your employer (and have not done so already), you should contact PMRS immediately to determine what benefits to which you might be entitled.
