Depending on the survivor option that was chosen at the time of retirement, will determine the benefits paid to the survivor. Please refer to the Certificate of Benefits form received from PMRS that details the selected survivor benefit chosen.

In the event of a member or retirees’ death, the survivors or beneficiaries should contact the employer or former employer to obtain the proper forms to claim any payable benefits.
